Scape – Hole Diameter On Hanger Bars

    15JUN26: There is some confusion with what was supplied when. Best described by LS in email to Brett Davis from Scape on 12JUN26.......

    Different hole diameter Hanger Bars – I recall discussing this with you some time back but I can’t exactly recall our findings at that time. I have undertaken some investigations this morning and this is what I can determine without having my team here for further input.

    1. Two separate POs issued by Scape
    a. 1129 – 3 pcs (I don’t have access to this PO in my email records and I am having troubles logging into Sam’s PC and I can’t see it in Sales@....)
    i. As I don’t have access to PO 1129, I cannot determine what hanger bar CAA DRG Revision is referenced.
    ii. CAA DRG CD4105 R1 – shows a 12mm dia hole
    iii. CAA DRG CD4105 Rev 2 – shows a 13mm dia hole
    b. 1150 – 17pcs (References CD4105 Rev 2 – 13mm dia hole)
    2. Findings
    a. Scape Non-Conformance Report dated 17/11/2025 states 2 out of 20 holes to be undersized and non-conforming. This appears abnormal due to the 2 PO qtys being 3 & 17 respectively. I recall this job and note that the 3pcs against PO 1129 were made and on site for a period prior to PO 1150 being issued and manufactured.
    b. Assuming the 3 pcs against PO1129 were made incorrectly against CAA DRG CD4105 Rev 1 (12mm dia hole) then this would explain the hole size non conformance but does not align with the non-conformance qty (2). It would be highly unlikely that CAA’s fabricator would have changed punch set part way through the punching of the hanger bar flat plates.
    i. It would be my best assumption that 3pcs were actually supplied as non-conforming against Scape PO 1129. Acknowledging that this differs to Scape’s non-conformance 20251117 report which states that all 20 hanger bars/holes were inspected and only 2/20 were found to be undersized.
    ii. My further assumption is that Scape PO 1150 was manufactured using a 14mm dia punch and therefore hanger bars (17pcs) were found to be conforming. Scape’s non-conformance 20251117 does not detail conforming hole dia from inspections undertaken.
    c. CAA DRG CD4105 Rev 2 is found to be in error as CAA do not have and to the best of my investigations have not had a 13mm dia hole punch. We have a 13mm dia x 31mm long elongated slot punch but not a 13mm dia round. We have a 12mm dia and a 14mm dia round punch.
    i. Resolution - I have asked for CD4105 Rev 2 to be amended to show a 14mm dia hole as that is the next upsize from 13mm available.
    ii. Once CD4105 Rev 2 has been amended, CAA to email copy of CD4105 Rev 3 to Scape for their records for any future orders.

    My apologies for not having formally closed out this CAR prior to now. I trust that this now affords you the opportunity to close this out internally and with the issuance of CD4105 Rev 3 (14mm dia hole) we should not experience a re-occurrence of the same non-conformance.


    Immediate action –SP to amend drawing CD4105, when back in office. Brett Davis to check discrepancy b/n quantities supplied with non-conforming hole diameters.
