Cullen Bay – Folded threads on bolts

    Source of issue –Customer: Cullen Bay Marina
    Description –

    We periodically supply CDZ8-20-175S with D Shackles welded to each end for attachment with Bolts which we supply machined with a conical tip. Refer to drawing CD4376.
    Gadian Dixon (Estate Manager, Cullen Bay), was preparing for the installation of the anodes. He tried threading the bolts (see image above) into the D Shackles. They would not thread.
    He sent a brief video to BL phone on 28th July 2026.


    Immediate action –We tried some SS bolts with him back in February. They worked well, so we sent them straight away so that their installation was not delayed.
    Root cause –Folded threads when conical point machined (YET TO BE VERIFIED)
    Corrective action –

    We will receive the bolts with what we believe is folded threads some time next week. We will check them. If there is an issue with the threads we will send them back to Graham at PPE to be fixed. We will then hold the bolts in stock until next time these anodes are required. Over the last 3 years they have ordered 50/year.

    Also, we should have checked these threads prior to dispatch. Training required to do this in future.
