Description –
16MAY25: Andre Sperling emailed photo of 1 x seriously cracked CDZ4-100D.
LS requested that the cracked anode be sent back to us for investigation and to inform our supplier, Metal Distributors.
Immediate action –LS replied, apologized and sent 2 replacements.
Root cause –TBC, need to discuss with Metal Distributors
Corrective action –
05AUG26: No re-occurrence of this issue. LS has not discussed with Metal Distributors. Close, but re-open if it happens again.
Verification –No re-occurence 12 12 months
